Watery eyes can make daily life very difficult. Beyond constantly carrying tissues, it is impossible to see clearly when your eyes are constantly watering. Just as our vision becomes blurry when we open our eyes underwater, when tears continuously accumulate on the surface of the eye, we cannot see our surroundings clearly.
Surprisingly, one of the most common causes of watery eyes is sometimes dry eye syndrome. When the surface of the eye becomes dry, the brain sends signals to produce more tears, which can result in the rapid production of low-quality tears.
Especially when we focus intently on a task, our blinking reflex decreases significantly. Tear secretion mainly occurs during blinking, so reduced blinking leads to eye dryness.
Prolonged use of computers or tablets, driving at high speeds, careful reading, or watching television can cause stinging, redness, and excessive tearing as a response to irritation. In such cases, consciously opening and closing the eyes and increasing the blinking rate can help.
Other common causes of watery eyes include tear duct obstruction, foreign bodies in the eye, eyelid margin inflammation, eyelid diseases, surface eye inflammation, ingrown eyelashes, and excessive exposure to sunlight.
Treatment for watery eyes depends on the underlying cause. First, a detailed eye examination is performed to identify potential reasons for excessive tearing. Under a microscope, the eye is examined for possible foreign bodies, infections, diseases of the cornea, dry eye conditions, problems on the inner surface of the eyelids, allergic signs, the condition of the eyelid margin glands, and eyelash root diseases. The openings of the tear ducts in the upper and lower eyelids are also inspected under the microscope. Further evaluation using eyelid laxity tests and dye tests is important for accurate diagnosis.
After a detailed examination, removing any foreign bodies, diagnosing and treating diseases that damage the cornea, and managing infections are crucial to prevent permanent damage. Surgical correction is possible for inward-turning eyelids or eyelashes.
In cases of inflammation or allergy, treatment targeted at the underlying cause will relieve symptoms. In many cases, simple measures recommended by your doctor—such as wearing sunglasses, changing makeup products, using warm compresses, or eyelid hygiene shampoos—can alleviate complaints.
If there is tear duct obstruction, continuous tearing can lead to severe crusting and even serious infections that may cause facial swelling. In such cases, surgical treatment is required.
Sometimes, these surgeries are exaggerated or misrepresented by physicians without sufficient experience, causing patients to avoid surgery unnecessarily. As a result, patients may have to live with persistent watery eyes and may experience severe infections during certain periods of their lives.
Ensuring that the physician you consult has experience in oculoplastic surgery will make the procedure short, lead to smooth recovery, and achieve a high success rate.
Lacrimal Duct Obstruction Examination
The best method for evaluating the lacrimal duct is through a lavage procedure, performed by an experienced oculoplastic surgeon using a cannula placed into the lacrimal duct. In this procedure, water is introduced into the tear drainage system from the inner corner of the eyelid, and the ease with which it flows into the nasal cavity is examined. It is very important to understand the location and extent of any obstruction at any stage with this procedure for treatment planning. In cases of mild blockages or small accumulations within the sac, this procedure can even be therapeutic.
Lacrimal Duct Obstruction Treatment
If watery eyes are caused by lacrimal duct obstruction, the treatment is surgical. There are three methods available today to create a new duct system.
The laser method, which has a popular name, is unfortunately the least developed and has the lowest success rate. In this blind procedure, soft tissue is excessively heated and burned, so the chances of reopening are very high, and it is never used in developed countries. Apart from this, the endoscopic method performed through the nose and the external method are high-success techniques.
The external incision method is preferred because it has a higher success rate in terms of controlling the entire anatomical structure. With improved cosmetic incisions, there is no risk of scarring, and it is the only method that allows successful intervention in problems at the entrance of the system.
In rare cases where the entrance part of the canal system is destroyed due to trauma, degenerative diseases, severe infections, or laser surgery, the glass tube surgery that can be performed by experienced surgeons is an option. Additionally, in these cases or in rare situations where excessive tearing is caused by excessive tear production, botox treatment applied to the lacrimal sac can provide encouraging results.
Treatment of Lacrimal Duct Obstruction in Children
In babies, alternative methods such as probing, balloon application, and tube placement are available for the treatment of lacrimal duct obstruction. These treatments are evaluated according to the patient's age, anatomical structure, and the severity of the infection.
Watery eyes are not only a condition that requires constant wiping with a handkerchief and is socially very uncomfortable, but it is also a serious problem that disrupts the patient's vision function. For both diagnosis and treatment, it is necessary to consult an oculoplastic surgeon who has specialized training in this field. In experienced hands, this condition can be resolved with medical or surgical treatments.
